I want to refer very specifically to the topic of therapeutic abortion.  I know that it has provoked a reaction in European countries, in the Nordic countries and above all I would say, in the Swedish government.   We even are given to understand that the withdrawal of the Swedish government has to do with this and other political factors, because, on the one hand, they withdraw from Nicaragua and other Latin American countries, it's true, but on the other hand, they continue offering aid to countries that are not among the most impoverished of Latin America, with the exception of Bolivia.But there are other countries that are in much better condition.  I think that Columbia and Guatemala remain.  I have to respect the criteria of the Swedish government, but I'm sure that if Olof Palme were alive, he would not agree with this decision.  It's ultimately a political decision.  They aren't content, the current Swedish leaders, with the fact that the Sandinistas have returned to power.  I am sure that the Swedish public has its own criteria.What is happening with this issue of therapeutic abortion?  This law was approved because of the Liberal Revolution, at the end of the last century.  I would say it was simply approved by the force of the Revolution, but, they are those measures that are taken without really having the approval of the population.  This happens with revolutions!  They are movements of general progress, but there are always acts carried out that aren't the best, such that the people are amenable to them.That was dormant.  When it came time to again debate the Penal Code and other topics in the National Assembly, it was, as it were, discovered that therapeutic abortion existed, and the debate began.It's important to also take into account the cultural elements of Nicaraguan society.  When we were in the middle of the Revolution of 1979, with the youth and women incorporated, there were comrades that proposed that we approve abortion, one step beyond therapeutic abortion.  We did profound surveys, among the people and the youth, and 99% indicated that they were against abortion.When they were asked if they had had abortions, some young women had had experiences with abortion, some women, in the interviews they said that yes, that they had had abortions, but they added that they weren't in favor of abortion, because they didn't feel good about having done it.We insist that it is necessary above all to change the quality of life of Nicaraguan families, of Nicaraguan women, to give training in the area of sexuality to the youth and not try to impose a law that isn't going to be approved by the population.To this it is necessary to add the political element.  In the 1960s, when the struggle of liberation in Africa, in Asia, in Latin america, gathered strength, in the middle of that great battle it began to be confirmed what was being promoted, developing birth control programs, because of the countries, the developed countries in conflict in those regions.  The United states, above all in Latin America, in Asia, Europe in Africa, in the struggle for independence by the African nations, came to affirm that the most serious problem was the excess of population.With so many people impoverished, the conditions were there for the multiplication of violence, the revolutionary struggle in all of those countries.  At that time, with a much smaller population in the world, in Nicaragua the population was at around one million eight hundred thousand inhabitants.Logically the proposal of therapeutic abortion lent itself to great confusion when all of these factors come into play.  And when the topic begins to be debated again, all of the profound inquiries we did, we were not in the government, but we did our surveys, 80% of the population was against abortion!  If we hold a referendum here on the topic, I'm sure that we would have a massive vote against therapeutic abortion.I think that this is a problem that has to be seen over the long term, that happens by elevating the quality of the population, happens by defending the rights of women and of children.The truth is that in Nicaragua, the level of domestic violence and violence against women, murder, etc., is the lowest in all of Central America.  Nicaragua has the best level of security at the Central American level.  The rates of violence, of criminality of every type, are much more elevated in the rest of the Central American countries.I think that this is a problem that has to be addressed over the long term, and I called for the health minister, because here, regarding this topic, some people have affirmed that, the law having barely been approved, scores of women have begun to die everywhere, a huge lie!  Here is the Health Minister, who can explain to us how this issue is being handled regarding the regulations and the whole Procedural Manual of the Ministry of Health. Minister of Health Guillermo GonzálezGood evening.  In effect, Comandante Ortega was explaining the complexity of the whole topic of therapeutic abortion.  I would like to give you objective information about this topic.Our country has a rate of maternal death, to give an indication, on the order of 95 out of 100,000 live births.  As I know not everyone is familiar with statistics: we are among the highest rates in Latin America.For our government, that is our principal problem, but when we come to break it down, to understand it thoroughly, we realize that more than 55% of our maternal deaths are linked to topics of female illiteracy, poverty, rural life, that is to say, those who live in the country.  That implies problems of access to communication, and problems regarding access to health services.For that reason the main policy of the government in the area of health was to overcome the economic barrier to health services.  Offering them free of charge has facilitated access for poor people so that these problems can be addressed.If we continue to analyze the maternal mortality data, we realize that another percentage we're talking about has to do with the quality of health services.  Therefore, our main problems have to do with resolving the poverty issue, the availability of services, and, above all, the quality of service.When it is discussed on a technical level…and we address it in international terms, the quantity of cases in which a medical decision had to be made, having to opt for one of the two, we realize that this percentage is minimal.  All of the cases that we have analyzed, their main problem was access to health services.Therefore, to address the problems related to the topic of therapeutic abortion, it isn't the central issue for our country.  We have thoroughly studied it over the last year and I want to tell you that with the measures we have taken, we have reduced the rate of maternal mortality by 10%, which indicates to us that we are on the right road.However, our health system also has the norms, the regulations, and the mechanisms to attend to what we call obstetric emergencies, which means that for every woman at high risk, it is our obligation to treat her in the health services.But I want to insist that the principal problem, priority number one, is to ensure that women have information and we are working on that with the Ministry of Education, aware that we have to prepare the people, with more information.We are also doing a lot of prevention work in the health centers, so that we can resolve the structural problem.  This year we are preparing personnel with better technical qualifications, taking them to the most difficult areas of the country, so as to reverse the problem at its roots.In the General Health Law, particularly, those juridical aspects are contemplated, in order to address the issue, like Comandante Ortega was saying, these particular aspects are not ignored, but rather the central aspects of maternal mortality are focused on.  I hope that I've given you a sufficient overview.The health system has its Procedural Manual, which requires doctors to implement technical and scientific practices, at the time a complex situation arises.  Specialists have been talked to, so that the system of guarantees for attending to all of these situations is managed according to technical-scientific criteria.  Thank you.Los Extractos Originales en Español "La democracia se realiza en un sistema parlamentario"  Texto Completo: http://www.presidencia.gob.ni/170108_DemocraciaRealiza.htmlDaniel Ortega, Presidente de NicaraguaQuiero referirme muy específicamente al tema del aborto terapéutico. Yo tengo que respetar el criterio del Gobierno sueco, pero estoy seguro que si Olof Palme viviera, no estaría de acuerdo con esa decisión. Es una decisión política al final de cuentas. No están contentos, los actuales gober-nantes suecos, con que el sandinismo haya regre-sado al Gobierno. Estoy seguro que el pueblo sueco tiene su propio criterio.¿Qué pasa con este tema del aborto terapéutico? Esta ley fue aprobada a raíz de la Revolución Liberal, a final del siglo antepasado. Yo diría que simplemente fue aprobada por la fuerza de la Revolución, pero, son aquellas medidas que están tomadas sin contar, realmente, con la aprobación de la población. ¡Eso pasa con las revoluciones! Son movimientos de avance global, pero siempre se toman acciones que no son las mejores, como para que la población esté apropiada de las mismas.Eso estaba dormido ahí. Cuando se vino a debatir de nuevo el Código Penal y otros temas en la Asamblea Nacional, como que se descubrió que en Nicaragua existía el aborto terapéutico y empezó el debate.Hay que tomar en cuenta también, los elementos culturales de la sociedad nicaragüense. Nosotros en plena Revolución del año 1979, con la juventud y las mujeres incorporadas, habían compañeras que planteaban que aprobáramos el aborto, un paso más allá del aborto terapéutico. Nosotros hicimos encuestas a profundidad, en la población y entre los jóvenes… y el 99% se manifestaba en contra del aborto.Cuando se les preguntaba que si habían abortado, si habían tenido experiencia de aborto algunas muchachas, algunas mujeres, en las entrevistas ellas decían que sí, que habían abortado, pero agregaban que no estaban a favor del aborto, porque no se sentían bien de haberlo hecho. Nosotros insistimos en que era necesario sobre todo, cambiar la calidad de vida de las familias nicaragüenses, de las mujeres nicaragüenses; darle formación en el campo sexual a la juventud y no ir a tratar de imponer una ley, que no iba a ser aprobada por la población.A esto hay que agregar el elemento político. En la década de los 60, cuando la lucha de libera-ción en África, en Asia, en América Latina, cobró una gran fuerza, en medio de aquella gran batalla se llegó a afirmar que se estaban promoviendo, desarrollando programas de control de natalidad, porque los países, las naciones desarrolladas en conflicto en esas regiones, Estados Unidos sobre todo, en América Latina, en Asia; Europa en África, en la lucha de la independencia de los países africanos, llegaban a afirmar que el problema de tanta lucha en estos países, era el exceso de población.Que al haber tanta población empobrecida, ahí estaban las condiciones para que se multiplicara la violencia, la lucha revolucionaria en todos estos países. En esa época, con una población mucho menor en el mundo, en Nicaragua la población andaba en un 1 millón 800 mil habitantes.Lógicamente el planteamiento del aborto tera-péutico, se presta a mucha confusión cuando juegan todos estos factores. Y  cuando se empieza a debatir el tema nuevamente, todas las encuestas de profundidad que hicimos, no estábamos en el Gobierno, pero hicimos nuestras encuestas, el 80% de la población ¡en contra del aborto! Si aquí hacemos un Referéndum sobre el tema, estoy seguro que tendríamos una votación masiva en contra del aborto terapéutico.La verdad es que en Nicaragua, el nivel de índice de violencia doméstica y de violencia contra la mujer, asesinato, etc., es el menor de toda Centroamérica, Nicaragua tiene el mayor índice de seguridad a nivel centroamericano. Los índices de violencia, de criminalidad de todo tipo, son mucho más elevados en el resto de los países centroamericanos.Yo pienso que este es un problema que hay que verlo en el largo plazo, que pasa por elevar la calidad de la población, pasa por defender los Derechos de la mujer y de la niñez.Yo pienso que este es un problema que hay que irlo viendo en el largo plazo y mandé a llamar al Ministro de Salud, porque aquí, alrededor de este tema, algunas personas llegaron a afirmar que apenas se había aprobado la ley, habían empezado a morir decenas de mujeres por todos lados ¡una gran mentira!  Aquí está el Ministro de Salud, que puede explicarnos cómo se está tratando el tema en lo que es el Reglamento y todo el Manual de Procedimiento del Ministerio de Salud.Guillermo González, Ministro de Salud   Muy buenas noches. En efecto, el Comandante Ortega, explicaba esa complejidad que está detrás de todo el tema del aborto terapéutico. Yo quisiera darles informaciones objetivas de este tema.Nuestro país tiene una tasa de muerte materna, para colocar un indicador, en el orden del 95 por 100 mil nacidos vivos. Como sé que no todos manejan el tema de indicadores, estamos entre las tasas más altas de América Latina.Para nuestro Gobierno, eso es nuestro principal problema, pero cuando lo llegamos a desagregar, a conocer a fondo, nos damos cuenta que más del 55% de nuestras muertes maternas, están ligadas a temas de analfabetismo de la mujer, de pobreza, ruralidad, es decir que vive en el campo. Eso implica problemas de acceso a la comunicación, y problemas de acceso a los servicios de salud.Por eso la primera política del Gobierno en el campo de salud, fue romper la barrera económica a los servicios de salud. La gratuidad ha facilitado el acceso de las poblaciones pobres para atenderse estos problemas.Si seguimos analizando los datos de la mortalidad materna, nos damos cuenta que el otro porcentaje del que hablábamos, tiene que ver con la calidad de los servicios de salud. Por tanto, nuestros principales problemas están en resolver el tema pobreza, la oportunidad de los servicios y sobre todo, la calidad de la atención.Cuando se discute técnicamente… y lo tomamos en términos internacionales, la cantidad de casos que debieron haberse tomado la decisión médica de tener que optar por uno de los dos, nos damos cuenta que ese porcentaje es mínimo. Todos los casos que hemos analizado, su principal problema fue el acceso al servicio de salud.Por tanto, abordar la problemática relacionada con el tema del aborto terapéutico, no es el tema central para nuestro país. Hemos estudiado a fondo durante este año y quiero decirles,  que con las medidas que hemos tomado, hemos reducido en un 10% la tasa de mortalidad materna, lo que nos indica que estamos en el camino correcto.Sin embargo, también nuestro Sistema de Salud tiene las normas, los reglamentos y los mecanismos, para atender lo que nosotros llamamos las emergencias obstétricas; lo que hace que toda mujer en alto griesgo, es nuestra obligación atenderla en los servicios de salud.Pero yo quiero insistir que el principal problema, la prioridad número uno, es asegurar que la mujer tenga información y eso lo estamos trabajando con el Ministerio de Educación, a sabiendas que tenemos preparar a la población, con más información.Estamos también haciendo mucho trabajo de prevención en las unidades de salud, de tal manera que nosotros podamos resolver el problema estructural. Este año estamos preparando recursos humanos, con mejor calificación técnica, lleván-dolo a las zonas más difíciles del país, de tal manera que podamos revertir de raíz, el problema.En la Ley General de Salud, precisamente, está contemplado esos aspectos jurídicos, de tal manera que el abordaje del tema, como decía el Comandante Ortega, no descuida estos aspectos particulares, pero sin embargo se enfoca en los aspectos centrales de la mortalidad materna. Espero haberles dado una panorámica.El Sistema de Salud tiene su Manual de Procedimientos, que hace que los médicos pongan en prácticas criterios técnicos y científicos, a la hora de una situación compleja. Se ha conversado con los especialistas, que el Sistema da las garantías para atender todas estas situa-ciones, de tal manera que se manejen con criterio técnico-científico.
